Assisted Care Rates
Based on Level of Care and Room Size
The Assisted Care area at St. Leonard is a 74-room
unit where residents can live independently and still have available
assistance on an "as needed" basis. The daily rates are listed
below. There is a $2,000.00 non-refundable entry fee. Admission is based
on an individual assessment of a potential resident, room availability
and financial qualifiers.

The Health and Rehabilitation Center at St. Leonard
is a 120-bed skilled and Intermediate care facility. It is staffed with
trained professionals who make every effort to see that our residents
are treated with respect and dignity. It is Medicare/Medicaid certified.

FEES: The repair, maintenance, or installation of any
item considered an option will be billed for parts and labor at

includes, but is not limited to, the following items: |

- Attic fans
- Humidifiers
- Water softeners, purifiers, or additional water heaters
- Laying wood floors in attic
- Extra shelves
- Extra walls, doors, and windows

- Changing light fixtures
- Storm doors (putting on, taking off)
- Hanging curtains
- Extra ductwork on furnace
- Adding extra circuits or outlets: electrical, cable or telephone.
- Laundry tubs

- Any changes in the landscaping (all changes must be approved)

- Changing paint colors of walls (interior only, no change permitted
outside)
