St. Leonard Foundation FactsFormed in 2003, the Foundation is a member of the Franciscan Services Corporation (FSC) located in Sylvania, Ohio and is sponsored by the Sisters of St. Francis of Sylvania, Ohio. The Foundation is a not-for-profit 501(C)3 organization (E.I.N. 32-0102715) located on the campus of St. Leonard senior living community, 8100 Clyo Road, Centerville, Ohio 45458 Fund management includes the following funds backed by the expertise of Fifth Third Bank, Dayton, Ohio: Greatest Need Fund, Alzheimer’s Fund, Assisted Living Fund, Benevolent Care Fund, Chapel Fund, Employee Emergency Fund, Employee Christmas Fund, Herman’s Fund (resident emergencies), Joseph Bernardin Residence Fund, Kreative Korner Fund, Library Fund, Nursing Care Fund, Rex Smith Memorial Fund, Special Projects Fund, Staff Development Fund, St. E’s Garden Fund, Activities Fund, Tate House Renovation Fund, Veteran's Fund and Franciscan Pilgrimage Fund. The Foundation has been endowed by the generosity of Province of St. John the Baptist in Cincinnati, Ohio, and the Sisters of the Poor in New York City, New York. St. Leonard Mission: Responding to the Gospel of Jesus, St. Leonard assist seniors of all faiths to achieve their highest level of independence. St. Leonard is:A continuing care retirement community with over 700 residents (age 62 and over), over 200 volunteers and over 300 full, part-time, and casual employees A 240 acre campus with a variety of living choices – converted from a seminary in the early 1980’s to a senior living center A Foundation committed to the raising and management of funds directed toward the support of its residents, staff, and facilities. Residents are not asked to leave due to lack of funds – thus, Foundation fund-raising activities are focused on the Benevolent Fund, designed to assist residents in need and keep them in the most independent living arrangement possible. Wide Range of Programs: Tai Chi, Pilates and a variety of exercise classes, Studio Art, active volunteer group, computer/internet classes, Veterans Organization, Kreative Korner Craft Center, art classes, woodshop, assorted acitivities and groups, libraries, beauty salon, bank, mini-mart, massage therapy, Sophie the pet therapy dog, as well as transportation to doctor appointments, groceries, and cultural events on and off campus Health Care and Rehabilitation Center: (nursing home) with 120 beds (18-bed Alzheimer’s unit) and the Holy Family Chapel providing daily services, scheduled activities and Rehabilitation therapy. Cottages: 175 units (159 2-bedrooms & 16 3-bedrooms), 1 car garage, appliances furnished, completely maintained interior, exterior and grounds, transportation and activities. Garden Homes: 75 free-standing homes (38 2-bedrooms & 37-3-bedrooms), 2-car garages, covered patio or sunroom, all appliances furnished, completely maintained interior, exeterio and grounds, transportation and activities. Joseph Bernardin Residence: 100 apartments designed for tax-credit, low income housing for seniors Chaminade Hall Independent Living: 80 apartments (3 -2 bedroom, 52 1-bedroom and Assisted Living: 74 private rooms with private baths and assistance with activities of daily living (Respite rooms available) Adult Day Care entitled Dayaway: a full service day program for seniors to enhance their social and cognitive abilities through mental and physical stimulation, maintaining their independence and dignity The Foundation’s mission is to energize the spirit of fraternal charity for the benefit of the St. Leonard community and others who are touched by our mission. We hope to foster a spirit of generosity to provide support for demonstrated needs. As we continue to grow as a Foundation, we are building funds that will meet the St. Leonard community’s changing needs and improve the quality of life for all those we serve. Benevolent Care-Costs annually over $400,000 mainly in the Assisted Care area where coverage is not provided by any government programs. St. Leonard offers continuing care and services to these residents, even after their resources are depleted.
