Position Summary
Under the direct supervision of the nurse in charge, the State Tested Nursing Assistant performs basic nursing duties to provide for the comfort, safety, and hygiene needs of residents. The State Tested Nursing Assistant assists residents in maintaining and possibly increasing their level of independence whenever possible.

Requirements
State Tested Nursing Assistant by State of Ohio or eligible for reciprocity. Must be willing to complete the Certified Dementia Practitioner course. Qualified candidates must possess a genuine heart for the aging population and an appreciate of a faith based environment.

Registered Nurse / Licensed Practical Nurse

Position Summary
Registered Nurse/Licensed Practical Nurse is accountable for the delivery of safe and therapeutic resident care from admission through discharge in collaboration with the physician, family, and other members of the multi-disciplinary health care team. Delivery of quality nursing care is assured through utilization of the nursing process. Dayaway Resident Assistant

Position Summary
The Dayaway Resident Assistant is responsible for providing assistance with personal care and activities for physically and cognitively impaired program participants. The Dayaway Resident Assistant may care for the following age-specific populations: adults and older adults.

Requirements
High School Diploma or equivalent required. Previous experience providing assistance with ADLs and working with physically and cognitively impaired adults preferred. Qualified candidates must be committed to our wellness philosophy and Franciscan values and possess an engaging personality.

Staff Development Nurse

Category: Part-Time

Position Summary
Working with the Staff Development Manager, the Staff Development Nurse assists with the administration of TB skin tests, Hepatitis B vaccines, and flu vaccines and assists with reminding employees who are due to complete their TB skin test or Sign and Symptoms form requirements. The Staff Development Nurse also tracks completion of mandatory in-services, flu vaccinations, and audits; assists with orientation; serves as the backup trainer for CPR and First Aid classes for employees; and completes exit interviews. Additionally, the Staff Development Nurse maintains the records of BBET, CPR, and First Aid certifications; assists with competencies for Nursing staff; completes paperwork for CPR and First Aid certifications; and provides other administrative support to the Staff Development Manager as needed.

Requirements
Current Registered Nurse in the State of Ohio with at least 4 years of experience. Proficiency with Microsoft Office. Ability to be flexible as schedule requirements dictate. Third Shift Nurse Supervisor

Category: Full-Time

Position Summary
The Third Shift Supervisor is responsible for assuring quality care for San Damiano Residence Assisted Living residents. The Third Shift Supervisor serves as a night-shift staff manager, maintaining standards of behavior and service objectives and standards of nursing practice. The Third Shift Supervisor’s essential duties include:

  • Monitoring resident care needs, evaluating staff delivery of care, and assuming clinical responsibilities in the absence of the LPN/RN staff nurse
  • Serving as a mentor and coach for the nursing staff
  • Staff evaluations and corrective action as needed
  • Maintaining effective communication with the Director of Nursing, nurse managers, and physicians
  • Documenting resident assessments, care plans, physiological and mental evaluations, and nursing notes and monitoring the quality and accuracy of the documentation
  • Following up on resident/family concerns
  • Participates in the on call rotation and assists by filling in for call offs as needed

Requirements
Associate or Bachelor degree in Nursing. Registered Nurse or Licensed Practical Nurse currently licensed in the State of Ohio. Currently certified in CPR. Minimum one year recent clinical experience related to gerontology nursing. Must possess a driven positive attitude and strive for high-quality standards. Must be available all shifts for the on call rotation and to fill in for call offs as needed. Long-term care experience and one year supervisory experience preferred. Administrator

Category: Full-Time

Position Summary
The Administrator directs the day to day functions of our 120-bed Health and Rehabilitation Center (HRC) in accordance with the current federal, state, and local standards, guidelines, and regulations governing long-term care facilities. The Administrator also assures that the highest degree of quality care is provided to our residents at all times.

In addition, the Administrator is responsible for the following duties:

  • Operating the HRC in accordance with the established policies and procedures of St. Leonard
  • Monitoring and improving the overall quality of services in Nursing, Environmental Services, Food Services, Maintenance, and Security at the HRC
  • Maintaining effective communication with interdisciplinary team, residents, families, physicians, vendors, and staff
  • Enhancing the continued viability of St. Leonard by advocating the Total Quality Improvement Process
  • Maintaining knowledge of current trends and practices in long-term care.

Requirements
Bachelor’s degree in Administration of related field required. Masters degree in Administration or related field preferred. Licensed Nursing Home Administrator in the State of Ohio. At least 5 years of management experience. Experienced in long-term care and short-term rehabilitation. Weekend Receptionist

Category: Part-Time

Position Summary
The Receptionist greets everyone who enters the Health and Rehabilitation Center, answers incoming calls, completes walking tours for prospective residents and family members, sorts mail for distribution, and assists other departments with paperwork as time allows.

Requirements
High school diploma or GED required. Must be able to work Saturday and Sunday from 11:00 a.m. to 7:00 p.m. Qualified candidates must be committed to our wellness philosophy and Franciscan values and possess an engaging personality.

Maintenance Technician

Category: Full-Time

Position Summary
The Maintenance Technician is responsible for ensuring the plumbing and fixtures remain in good working order; performing maintenance, repairs, and installation of electrical systems and components, HVAC equipment and systems, refrigeration, and appliances; and completing other general maintenance tasks.

Requirements
