ST. LEONARD ASSISTED LIVING - Special Care
LEVELS OF CARE:
ADMISSION AND LEVEL OF CARE DETERMINATION WILL
BE BASED ON AN ASSESSMENT OF CARE NEEDED FOR EACH RESIDENT. ASSESSMENTS
WILL BE PERFORMED BY ST. LEONARD NURSING PERSONNELL.
LEVEL II
• Minimal assistance for individual with cognitive
impairment that requires the presence and/or
guidance of staff on continuing basis for the purposes of supervision to prevent
harm to self, to
achieve their highest practical level of functioning.
• Daily cueing and/or re-direction for ADL's, attending activities, dining,
etc.
• Requires daily assistance on scheduled basis for personal health care
(grooming and dressing)
• Needs some assistance with toileting and/or managing incontinence
• Needs assistance in dining room with meal (obtaining items from salad
bar, set-up, etc.)
• May need escort/transportation to and from meals, activities, beauty salon,
etc.
• Daily assistance with bathing
LEVEL III
• Behavior management with cognitive impairment
that requires the presence and/or guidance of staff on a continuing basis
for the purposes of supervision to prevent harm to self to achieve their highest
practical level of functioning
• Needs consistent cueing and/or re-direction for attending meals, activities,
etc.
• Needs assistance on scheduled basis for all personal health care
• Needs to have toilet/incontinence care on scheduled basis
• Needs transportation to dining room, meals, or other scheduled activities
• Needs Resident Assistant for bathing, either in own room or in tub room
• Needs monitoring for maintaining nutritional status
