ST. LEONARD ASSISTED LIVING
LEVELS OF CARE:
ADMISSION AND LEVEL OF CARE DETERMINATION WILL BE BASED ON AN ASSESSMENT OF CARE NEEDED FOR EACH RESIDENT. ASSESSMENTS WILL BE PERFORMED BY ST. LEONARD NURSING PERSONNEL.
LEVEL I
Activities of Daily Living (ADL)
- Capable of ambulating or transporting self to the dining room
- Can ambulate short distances or can use the walker/wheelchair independently for long distance
- Transfers in/out of the bed and in/out of the chair independently
- Capable of bathing self or may require assistance with back and lower extremities
- Care for self at toilet completely; no incontinence/manages incontinence
- Dresses and undresses and selects clothes from own wardrobe
- Eats without assistance
- Normal-oriented to time, place and person. Able to plan daily events.
- Medications may either be self-administered or administered by the licensed staff
Independent Activities of Daily Living (IADL)
- Able to look up phone numbers, dial numbers and answer the phone
- Has the ability to access St. Leonard’s transportation system
- Shops without help and purchases both large and small items
- Capable of filling out menu, select items from buffet and carry them to their table
- Performs light tasks (i.e. making bed) maintain acceptable level of cleanliness
- Manages financial matters
- Does personal laundry
LEVEL II
Activities of Daily Living (ADL)
- Needs help getting in and out of shower/tub. Can wash face and hands but cannot wash the rest of body
- Dresses and undresses self with minor assistance. Needs assistance in selection of clothes
- Ambulates with assistance of assistive devices
- Grooms adequately. Requires supervision
- Independently manages incontinence. May require assistance 1x/week to manage incontinence
- Eats without assistance. May require food to be cut
- May have some fluctuation in memory but can plan daily events appropriately
- Medications Administered
Independent Activities of Daily Living (IADL)
- May be able to answer phone or dial operator in an emergency
- May be able to shop, but requires assistance and someone to accompany them
- May require assistance in carrying food from the buffet and filling out menu
- May require help with bed making and straightening room
- May be able to launder small items, other items completed by staff or family
- May be able to manage daily buying needs, requires help in managing checkbook and paying bills
- May travel but needs someone to travel with/cannot make transportation arrangements
LEVEL III
Activities of Daily Living (ADL)
- Does not wash self, but is cooperative with those who bathe him/her
- Requires major assistance in dressing but cooperates with efforts of others to help
- May need escort/transportation to and from dining room and activities
- Needs total grooming care, but can remain well groomed after help from others
- Needs assistance getting on and off toilet. Requires assistance to manage incontinence daily
- Feeds self with moderate assistance and is untidy
- Frequent fluctuations in memory and orientation causing inability to plan daily events. Requires frequent cueing. Judgment impaired
- Medication administers
Independent Activities of Daily Living (IADL)
- May be able to answer phone, unable to call out or use phone book
- May be able to go to store, but requires assistance in decision making for items. Family does shopping for resident
- Unable to do laundry
- Unable to manage money
- Unable to travel without escort/cannot make any travel arrangements
Special Care Level II
Activities of Daily Living (ADL)
- Requires escort and redirection for going to the dining room, activities, ADL’s and beauty shop
- Requires staff assistance or supervision with bathing. Washes face and hands only, but cannot bathe the rest of their body
- May be continent but requires frequent reminders and requires assistance in cleaning self. Soiling or bedwetting while asleep, more than once a week
- Dresses and undresses self with minor assistance. May require assistance in selection of clothes
- Grooms self adequately with occasional assistance or supervision in grooming
- Feeds self with moderate assistance
- Ambulates independently
- Frequent fluctuations in memory and orientation causing inability to plan daily events; judgment impaired. Requires frequent cueing
- Medication administered
Independent Activities of Daily Living (IADL)
- Able to answer phone, may require help in dialing out and obtaining phone numbers
- Requires help in making decisions on items while shopping
- Cannot make any transportation arrangements/may have difficulty in recalling areas in building and on campus
- Can fill out menu/may need assistance at the buffet, remains tidy in dining room. Social and etiquette skills remain intact
- Unable to manage finances or checkbook
- May be able to launder small items, wash out socks, stocking, etc. Staff or family to complete other laundry
Special Care Level III
Activities of Daily Living (ADL)
- Does not wash self but is cooperative with efforts of those who bathe him/her
- Requires major assistance in dressing but is cooperative with efforts of other to help
- Ambulates with assistive devices
- Needs grooming care and assistance but remains well groomed after help from others
- Requires assistance to manage bladder continence and requires help in keeping clean. Soiling or bedwetting while asleep, more than 2x/week
- Requires assistance in dining room; may require that food be cut up and packages opened. May require cueing but can feed self, and is untidy
- Requires monitoring for maintaining nutritional status
- Requires assistance in getting to and from activities, dining room, beauty shop
- Evidence of impairment in short and long term memory. Difficulty in following directions, holding cues or expressing needs OR safety risk present
- Exhibits behavior responsive to intervention and behavior management program with a frequency of less than 2x/week but less than daily OR a behavior management program with a frequency of daily or multiple times per day
- Medication Administered
Independent Activities of Daily Living (IADL)
- Unable to use the phone independently
- Unable to shop independently
- Unable to fill out menu without assistance and obtain food from buffet
- Unable to do any light daily tasks and does not recognize the need for
- Unable to manage money
- Unable to do personal laundry
